• New Club Information Form (PDF) : This form is used to alert Kiwanis International within 24 hours of a new club’s formation. This is the PDF version to download, print, complete and fax or mail to the international office (instructions on the form).

• New Club Information Form (MS Word) : This form is used to alert Kiwanis International within 24 hours of a new club’s formation. This is the Microsoft Word fillable-field version to complete (type in data, tab through fields, save to your desktop) and then e-mail to the international office (instructions on the form).

• Member Information Form - This downloadable (PDF) form is used for adding or deleting members, and changing member information. The form should be printed out, completed, and then mailed to Kiwanis International (address provided on the form).

• New Member Application - Have new members fill out this form when joining your club. The application is a downloadable PDF (to print, complete and then keep on file with your club records).

• Recruitment Brochure - Order the ‘Changing Tomorrows Today’ recruitment tool for 1:1 discussions with prospective members. The brochure also contains a member application on the back page.

• How to find your Kiwanis membership ID - Reminder page listing the three ways you can find or provide a Kiwanis membership ID number.

• Life Member Status Application - This downloadable PDF form is used to request lifetime Kiwanis membership status for an active Kiwanian. The form should be printed out, completed, and then mailed to Kiwanis International (address provided on the application). The application also provides details on eligibility, benefits, and limitations.

• IRS Information - A new form is available for clubs who were not required to file a tax return.
